The person submitting this enrollment, on his or her behalf and behalf of such children and all personal representatives, assigns, heirs, and next of kin thereof (“Releasing Party”), hereby releases, waives and discharges each of St. John Lutheran Church and School and St. Paul's Lutheran Church of Round Lake Park and each of its ministry councils, boards, employees and agents (each, a “Released Party”) from all liability to any Releasing Party, and assumes full responsibility and risk, for any loss or damage, and any claim or demands therefore on account of bodily injury, death or property damage, whether caused by Released Party’s negligence or otherwise while any Releasing Party is participating in any program affiliated with Released Party. He or she further expressly agrees that the foregoing release and waiver is intended to be as broad and inclusive as is permitted by the law of the State of Illinois and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. *
